- When Kaoru Inoue (Junichi Okada) opposes Soichiro Ogata (Shin'ichi Tsutsumi), Soichiro makes the cryptic remark "It couldn't be helped. It was for a greater cause ..." One month later, at an event hall in Roppongi Hills, there is a charity event for landmine eradication. Kaoru does security with Security Police officers Soichiro Ogata, Eri Sasamoto (Yoko Maki), Takahumi Yamamoto (Satoru Matsuo) and Mitsuo Ishida (Yu Kamio). Kaoru Inoue senses a threatening existence and starts to suffer a severe headache and becomes dizzy. When Kaoru was young, his parents were killed by terrorists and that incident has had an traumatic effect on Kaoru since. Inside the event hall, Kaoru Inoue replies to Ogata calls that everything is fine. The Secretary of Japan then appears on stage and Inoue again senses a threatening existence. Kaoru has a premonition of what will happen in the future. Right after, Kaoru finds a man in a suit holding an umbrella, even though the weather is clear, and starts running towards him. The man approaches the stage and Kaoru is blocked from reaching him by other spectators. Kaoru Inoue warns Eri Sasamoto about the man. As the man attempts to push a button on the umbrella, Kaoru and Eri are able to restrain him and take the umbrella away. The man slips away and runs off. Kaoru Inoue chases the man to the center of the city and arrests him near the subway platform. The chase was covered by the media. The chief of SP, Yoshiharu Nakao (Shingo Egami), later stares down at Kaoru and tells him a pipe bomb was discovered in the umbrella which could have caused severe damage. Prime Minister Yuzou Asada (Kei Yamamoto) and other higher government officials gave Inoue favorable reviews, but Kaoru, nevertheless, crossed proper protocols and should refrain from such excessive behavior in the future. That evening, a campaign fund raising event is held in support of of Secretary General Kunio Date (Teruyuki Kagawa). After the fund raising event, the secretary for the Ministry of Defense Takikawa (Takehiro Hira), who was a member of the same university political club that Soichiro Ogata attended at Tokyo University, chief international terror agent at the Ministiry of Foreign Affairs Anzai (Kazuki Namioka), Metropolitan Police Department member Mitsuhiko Kaziyama (Satoru Date) and Soichiro Ogata gather together in a suite room of the hotel with Kunio Date. There Takikawa tells Soichiro Ogata that its time to perform the plan. Takikawa affirm that the soft landing way pushed by Yuji Nishijima (Kisuke Iida) will not be effective to build up the country. Anzai insists a crisis is needed to change people's values. Kunio Date promises to support them and orders Soichiro Ogata to carry out their plan. Around that time, Ichiro Tanaka (Toru Nomaguchi) investigates the relationships between Kunio Date, Kunio Date, Kaziyama and the suicide of government official Yuji Nishijima. A few days later, Soichiro Ogata calls Kaoru Inoue at night to the meet at the park. The park was the same place where tragedy met Kaoru Inoue's parents some twenty years ago. Soichiro Ogata begins to tell a shocking truth, while Kaoru Inoue can't understand his intentions. The next day Soichiro Ogata assigns to Security Police officer in the #4 Security Section an assignment to guard Kunio Date. After guarding Kunio Date that night, the terrorist plot finally begins attacking Kaoru Inoue and his colleagues.

- Having noticed a slight change in the photo of his fiancé and him, Jin wonders if saving Ryoma has altered the future. He decides to try to stay out of events in the village to prevent anymore changes. However, as cholera sweeps the village, the medicine men call upon Jin to give them the cure. He tells them that he does not know. As hard as he tries to stay neutral, Jin's decision is challenged when Kiichi, the son of the woman he saved, becomes ill. Saki confronts him, as he asks her to return to her home. However, his fondness for the boy, changes his mind. He begins to educate the village and treat the ill. Saki, unable to stay away, goes to the Western medicine office to ask Koan and the others for their help. With distrust, since that morning Jin told them that he knew of no cure, they arrive at the quarantined home with disbelief. He tells them the cure, which seems too easy and contrary to the western medical book Koan had translated. For them to believe him, Jin is asked as to where he has acquired his knowledge. He cannot say, so they begin to walk away. Yet, as Saki runs after them, one of them becomes ill. Koan decide to believe in Jin.
- Now that Jin has Koan Ogata, the supervisor of the Western Medicinal Office for the village, and his team on his side, Jin begins to create a large-scale plan. He asks if rubber hoses exist and if a syringe needle can be made to create an IV drip. The town begins to make saline solution, sterilize everything with alcohol, and burying waste and burnt clothes in large ditches. Ryoma takes it upon himself to be the leader of waste disposal. Jin teaches the doctors how to administer an IV drip. Kiichi and Yamada recover. Jin suddenly becomes sick and has delusions while unconscious. Tae runs to summon Ogata. Saki performs a technique that Jin had taught her, earlier that day. To administer more fluids into his body, she puts an IV drip into his femoral artery. Ogata wants to do it, especially since the artery is in the groin, but Saki is insistent that she is Jin's doctor now. Kyotaro is dispatched to educate other villages of the cure. Koan Ogata goes to the president of the western medicine offices to ask him to educate all offices on how to cure cholera. Jin recovers and the epidemic has ended. Ogata asks Jin, as his next mission, to tackle the Western Medical Book and correct what is incorrect. He is also asked to lecture on his knowledge of medical procedures. Jin accepts. In the last minutes, we, once again, are privy to an unknown courtesan. Who is she?
- It is now winter of 1862. Jin had confessed to Saki that he is from the future. However, she has gone about as if nothing has happened. Kiichi is an apprentice at Daikichi's shop. Ryoma has become Katsu's student, while Kyotaro is his secretary. Jin is lecturing at the Western Medical office, with Saki in attendance and Ogata as his most fervent and out-spoken student. One day, as Jin is creating a tool that could drill a hole in a patient's skull, he and Saki get too close. She jumps away and asks him what he wants for dinner. She agrees to make him a special dish. Her mother scolds her for being obsessed with medicine, when it might interfere with her future marriage opportunities. Later that day, Ryoma asks Jin out for dinner. Saki is disappointed, since she made a special meal for JIn. Ryoma's surprise for Jin is a visit to the red light district. Jin does not consider that a treat and is immediately hesitant. Ryoma had heard that the top courtesan, who had previously rejected him, is seeking a doctor. Believing that he would score points with her by bringing the best doctor to her, he even wrestles Jin so as to keep him in the room. Courtesan Nokaze enters. Jin is shocked at her resemblance to Miki, his fiancé in the future. He wonders if she is a descendant of Miki. She sticks out her tongue and shows him the whites of her eye. When he says nothing, she asks them to leave. As they leave, Jin asks one of the men working there to relay a message to her. He is the first doctor to diagnose her anemia. She rushes to stop them and takes him to Oyaji, the proprietor of the brothel house. Jin diagnosis him. He needs to relieve the build up of blood in his head. He explains what he must do. Oyaji's wife's reaction is disbelief and horror. Courtesan Nokaze rushes in to vouch for Jin. She even agrees to attend to a burly man, she has rejected many times in the past, if he fails. Kyotaro is summoned to bring Jin's surgical supplies. As Saki gathers his things, she stumbles upon Jin's photo of him and Miki. As Jin operates, Ryoma sits with Courtesan Nokaze and asks her why Oyaji is so important to her. She tells her story of how she was sold at ten years of age to the brothel, with promise of plenty of food. As she discovers the truth about the house, she decides to disguise herself as a boy, by cutting her hair, and run away. She enjoyed the entire day until night came and hunger set in. Feeling defeated, she returns to the house. Expecting to be punished severely, she is shocked to have Oyaji comfort her and give her plenty to eat. He later gives her a pep talk about a possible future where she could become the top courtesan and make her own choices. She sees him as a father figure. The operation is successful, even though Jin had to drill two holes, since he does not have the luxury of an MRI or x-ray machine. Courtesan Nokaze offers Jin a personal session with him at a later time. As Jin stutters, Ryoma tries to get a session with her, but she calls him a hick. Jin goes home and takes out his photo to see if it had changed. It has. Miki is no longer in a hospital bed, while the date of October 13, 2007 is the same. When Jin returns to check on his patient at the brothel, Saki goes along. There she comes face to face with Courtesan Nokaze. She immediately sees the resemblance from Jin's photo. Jin is offered to return for a meal.
- After the check-up with Oyoji, Courtesan Nokaze asks Jin to see another patient. He discovers a former courtesan in the last stages of syphilis. There is no cure, since penicillin has not yet been discovered. Jon tries to persuade the rest of the prostitutes at the brothel to accept to an examination for signs of syphilis. However, they call him a pervert and refuse. Feeling defeated for his inability to help Courtesan Nokaze, Jin suddenly recalls the first meeting with Miki. She presented him with the procedure to create penicillin. He immediately seeks the help of the staff at the Western Medicine Office and creates a penicillin factory. Even with the inconsistent results of the process, he creates an effective dose to administer to the former courtesan. Her physical appearance is healed, yet she was too far along in the illness to be saved. It continues to be a slow process to create and administer the cure to the rest of the prostitutes at Yoshiwara. Even with the discovery of penicillin, Jin still feels defeated that he could not save the one person Courtesan Nokaze asked him to. Saki surprisingly asks him if someone in the future discovered penicillin. He shares his photo with her and relays the story of how Miki educated him on creating penicillin. With Jin making so much medical history, there is uneasiness in the hierarchy of the Western Medicine Offices.
- With Jin's growing popularity, the Study of Medicine Office find him a threat to their livelihood and status as the medical authority. A surgical instrument of Saburi's is found in the room of a murdered prostitute. The doctors at the Study of Medicine Office use it to their advantage to accuse Saburi of murder. He confesses that he's had an agreement to do autopsies at the Yoshiwara in exchange for a proper burial for the prostitutes. Ogata takes responsibility and resigns. However, Jin decides it would be best if he resigned instead to keep Ogata as head of the Western Medicine Office. Regardless, the president, fearing of becoming obsolete, orders the assassination of Jin. Courtesan Nokaze hears of the growing rumor of the assassination and sends a message to Saki for her help. Saki rescues Jin. Trembling, Jin finally feels alive and not in a dream state. Ryoma, hearing of the attempt, confronts the president, who does not deny it. Ogata and his staff find the penicillin factory destroyed. Ogata begins to cough up blood. Jin realizes that the photo hasn't changed since he produced penicillin. However, as he tucks it away, he discovers a coin made in 2010.
- It is 1863. Jin recognizes that it is the beginning of the Bakumatsu period. Ryoma, (a prominent historical figure who fights for the creation of a modern government, to prevent colonization), has been sent to build a strong navy. Kiichi's mother, Tae, is severely burnt by hot oil. Jin is summoned and prepares to do a skin graft. He asks Ogata for lots of penicillin to prevent infection. Since the factory was destroyed, Yamada has been in charge of restarting the production. Ogata invites a friend, Hamaguchi, to observe the surgery. As the surgery is about to end, news comes that the factory has burned to the ground. The war between eastern and western medicine continues. Jin tries to produce penicillin on his own with little success. Like a miracle a jar of penicillin shows up for him. Tae recovers. Jin asks where the penicillin came from? A secret factory is revealed to Jin. Hamaguchi, who observed him operating, is a member of the Yamasa Family. They manufacture soy sauce. A small warehouse that used to be used to make soy sauce was converted into a penicillin factory with the funding of Hamaguchi. Ogata had sent him several letters and visits to persuade him to invest in the future of medicine. Hamaguchi also informs Jin that Ogata has a severe case of consumption. He rushes to help. Ogata allows him to examine him, only to confirm the diagnosis. Jin and Ogata have an intimate moment. Ogata asks Jin if he is from the future and if medicine brings peace to the world. Ogata dies. Ryoma sends a letter to Jin concerning his destiny. He desires to bring the faction that is pro-isolation and the one that is pro-trade and modernization as one. That inspires Jin to create a hospital that unites eastern and western medicine.
- The prostitute is feeling a lump in her breasts and requests the doctor to examine her as she can be only be married off upon receiving a clean bill of health. In the meantime, Miki meets Nokaze and insists that they are both in love with the same man. The doctor has opted to leave Saki's house, which angers and disappoints Miki.
- The doctor faces a dilemma and struggles with his prognosis. The friendship and professional input of two others help him make a decision. In the meantime, Saki is confronted with the reality of a marriage proposal and has to come to grips with being unable to be a nurse for Minakata. The rogue samurai disappears after taking a tumble and he is feared dead. Finally, the clan lord is unhappy with the fate of his pending marriage.
- Kyotaru sells his late father's treasured tea cups to buy a pair of glasses for Hatsune, a prostitute he first met while guarding Jin at Yoshiwara. With Ogata dead, Jin must prove his worthiness for Hamaguchi to fund making a more potent penicillin. Jin goes to Yoshiwara to see another ill prostitute, Hatsune. A local doctor performed an abortion that caused blood-poisoning. The penicillin is not potent enough. They must try to find immediate funding for a new production a much more potent dose. A male actor, who plays theatrical female roles, is asked to loan the money. He refuses. Ryoma goes to the local doctor who performed the abortion. The man quickly signs a contract with him for the full loan. The penicillin is made and Hatsune is given the potent penicillin. Seven days later, the abortion doctor shows up requesting that the full amount must be paid or he will take the potent penicillin, which was put up as collateral. He cheated Ryoma by telling him that he would have seven years, not seven days, to repay the debt, interest-free. Everyone is desperate to find a solution. Kyotaro returns to the actor and tells him that he will sell his body to him. He agrees to the loan only if Kyotaro publicly bows before him and begs for money. As a samurai, that is unheard of and humiliating. Hamaguchi decides to support Jin.
- The doctor receives an invitation to the brothel where a surprise awaits him. yet despite being asked to arrive alone he has another man in tow. At the same time a fire breaks out, and remembering his earlier promise to the local thug, the doctor sets out where his ingenuity and skills are just in time and quite handy. With Saki's help the doctor tends to the patients amidst a raging fire all around, but he has left a disappointed prostitute behind all the same.
