He had suffered a severe heart attack four years before he had another, dying in his sleep at home.
He was a chain smoker and heavy drinker who never took exercise, ate indiscriminately and worked long hours at a frantic pace. He was once described as "a heart attack waiting to happen", and did die of a heart attack at only 51.
