- A teenage boy is impaled with a shotgun. Although it is onscreen, the lighting is very dark and it's hard to make out any gory details.
- In the opening scene, a man is thrashed around and slammed into the ground repeatedly by a Pumpkinhead. The creature then holds up the man and it's difficult to tell what happens next, but it looks like he snaps his neck.
- A teenage boy is pulled away, and killed offscreen. We then see his mutilated corpse. It is scratched, chomped, and covered in blood.
- Pumpkinhead carves an upside down crucifix onto a religious teenage girl's forehead.
- Although the kills themselves are usually offscreen, the bloody aftermaths are often shown. It can be very disturbing.
- Human on human violence includes very brief shootings and beatings.
- Pumpkinhead smashes a teenage girl's head through a window. Blood oozes out of the wound.

- The creature known as Pumpkinhead looks very grotesque. It's gigantic and has big white eyes with no pupils. It has a large deformed head and putrid body.
- The creature may be frightening to younger viewers. Other than that, not many frightening elements other than a few jump scares.
- The whole movie has a creepy, atmospheric feel.
- The overall premise of the film is very dark, and the themes of revenge are distressing.
- There are lots of frightening images.
- Pumpkinhead is not as gory as many other R-rated creature feature features, but still creepy and violent

Violence & Gore
- A 9 year old boy is run over by a motorbike and dies. When we get a closer look at the boy, some blood is shown.
- A teenage boy is beaten with a blunt object by another teenage boy and it is unknown weather he dies or is rendered unconscious.
- It is revealed that in order to kill Pumpkinhead, the person who summoned him must die. The man shoots himself, but he survives. He tells a teenage girl to kill him and end Pumpkinhead's rampage. She then shoots him several times until he dies. Pumpkinhead dies along with him, before he engulfs in flames and disintegrates.
- Pumpkinhead throws a teenage boy off of a tree, and he is impaled by another tree.
Frightening & Intense Scenes
- The summoning scene in which Pumpkinhead is summoned may frighten some viewers.
