- Riki-Oh punches an obese prisoner in his stomach, and his fist penetrates the abdomen and slice it open, and it begins shooting copious amounts of blood and guts, as the fat man falls on his back, screaming in agony.
- Two prisoners throw a middle-aged inmate at a window. He gets a bladed device and runs at an inmate, but gets tripped, and another prisoner catches the blade and slices the man's face. He screams in agony, as his nose becomes mutilated and covered in blood.
- Riki-Oh punches a prisoner and splits his head in half, and the head falls on the ground and brain matter falls out.
- A teenage girl throws herself from the roof of a building and falls to her death.
- Riko-Oh is imprisoned in a circle surrounded by thick metal spikes, and tortured. He is repeatedly hit on his head and face, then razor blades are shoved into his mouth and taped off, and he gets slapped several times, and the razors start to graphically stab his cheeks from inside his mouth, and his face becomes covered in blood. He later spits the razors out, and they land on the warden's face.
- A dog is violently kicked and it splits in half and dies. The severed upper half of the dog is shown, with blood and intestines oozing out of it.
- Riki-Oh fights a mutated prison warden; he punches through his stomach but gets strangled and violently thrown around, then he crushes one of the warden's knees and punches through his chest, and picks him up and throws him in a meat grinder, and the warden lands on his legs, and while the machine is grinding his legs, he tries to strangle Riki-Oh but fails, and gets his body forcibly pushed deeper into the grinder, with copious amounts of blood and guts splattering everywhere, as the warden's body turns into a big pool of blood and minced meat.
- Riki-Oh gets shot a few times with a gun, but he survives and jump-kicks his shooter in the knee, and punches his head, graphically crushing both.
- The assistant warden stabs Riki-Oh's hand and pins it down on his desk with his metal hook for a hand, and blood starts to ooze out on the desk, then he proceeds to punch Riki-Oh a few times, and his cheek and lips start to bleed.
- A prisoner swings a knife which slices the skin off an inmate's face from nose to chin. Later, the dead inmate's skinned and mangled corpse is seen in graphic detail.
- Riki-Oh is attacked by another prisoner while trapped in a cell. Riki-Oh's arms are secured with the cell door's steel bars, then he's repeatedly punched, and starts to bleed from his mouth, then his opponent tries to crush his head by squeezing it between two bars but fails, and Riki-Oh punches him in the jaw, and his fist graphically penetrates the mouth, then punches his fist, completely and graphically crushing it.
- The prison warden stabs an inmate in the eye with his walking stick. The stick is seen stuck in the inmate's eye, and blood is seen covering it.
- Riki-Oh punches the assistant warden in the eye, which causes the eyeball to pop out of its socket.
- The prison warden shoves a prisoner's hand into a meat grinder, which completely minces the arm, then he pulls it out of the grinder, and it starts shooting lots of blood.
- The assistant warden kills an inmate, offscreen, and drags his dead body from the mouth with his hook arm, and the prisoner's mouth is covered in blood.
- One prisoner breaks a glass bottle and stabs the assistant warden in the neck, then he chops off his right hand with an ax, with lots of gore.
- The prison warden shoots the assistant warden with a special bullet, which inflates his body like a balloon and he violently explodes.
- A prisoner gets his leg stuck in a boiler, and Riki-Oh uses his bare hand to graphically amputate both of the inmate's legs.
- An inmate gets tripped and his face falls onto a wooden plank of needles that penetrate half if his face. As he lands, blood seen pouring out in a second.
- The prison warden shoots a prisoner, who's trying to escape, with one of his special bullets, and he violently explodes.
- An inmate shows his mouth and his tongue has been cut off. No graphic detail.
- A man's corpse is seen with his back graphically skinned, and the bloody skin is hung out to dry.
- A prisoner graphically crushes another prisoner's head with his bare hands, and the head explodes, and another prisoner is stabbed with a metal rod, and blood starts to shoot out from the wound.
- A prisoner wielding a large saw strikes another inmate on his face, and the saw goes deep into the inmates' cheek and head, and blood starts to seep out, then the assistant warden drags the inmate by the cheek with his hook-hand.
- Riki-Oh fights two prisoners, one is wielding two large metal pins, controlled by long ropes, and is used to repeatedly stab Riki-Oh on his chest and arms, then the ropes are used to constraint Riki-Oh's hands.
- Riki-Oh punches a prison guard through his shield and through his torso, and blood explodes from guard's back.
- An inmate fights against a prison head. He gets his arm punched with blood spitting out, he gets punched through his jaw and his fingers are punched off, with gory details.
- A prisoner is seen hanging from a noose by his neck, dead.
- A prisoner attacks Riki-Oh with a large nail, which he stops with his open hand, and the nail goes all the way through it. Riki-Oh then closes his hand, graphically mangling the fingers of his attacker, then removes the nail, which oozes blood, and then punches the prisoner, exploding his body.
- Riki-Oh is fighting another prisoner wielding a knife; he gets part of his arm graphically sliced open, but punches his opponent on the back of his head, causing one of his eyeballs to fall out, then graphically fixes his open arm wound; his opponent slices his own abdomen open with the knife, graphically pulling out some of his intestines to try and strangle Riki-Oh with them, but gets thrown in the air and violently punched on his neck, immediately killing him.
