- Like you could expect there are many graphic and disturbing scenes in this fourth part.
- The video starts with actual images of various corpses, a few close ups of hands and feet can be seen.
- There's footage of the process of an cremation which is extremely disturbing, the skin of the corpse can be seen boiling (including blisters which erupt from the skin).
- Staged footage is shown of a mental patient who killed a few doctors offscreen, he threatens a nurse with a screwdriver, finally he stabs her in her throat. Blood can be seen running out of the mouth of the woman.
- The corpse of a woman can be seen who got electrocuted on the railroad.
- The stiff corpse of a man who got shot in the back of his head can be seen.
- Footage is shown of a mangled man who got pinned between a subway train and the platform, it is told that the train worked as a large tourniquet as long as it stood still. When the train moved his blood and internal organs spilled out of the gaping wound (this is not shown).

- The result of a failed bombing attempt can be seen, the terrorist has both of his legs twisted. His hip is a bloody mess.
- A staged clip shows a bank manager and a robber being locked up in a safe, the robber shoots the manager in the face. A few moments later the security camera shows the robber suffocating to his death.
- The staged electrocution of a bald headed woman can be seen, it isn't that disturbing since you'll notice that it's staged.
- A staged clip shows a man having his arm bitten off by a tiger.
- Footage is shown of insects eating flesh and cleaning the bone.
- Disturbing footage is shown of a vietnamese family who slaughter a puppy, a woman lays the puppy on a carving board and proceeds to cut the puppy open and disembowels it. NOTE: This footage could be staged since you never see the actual slaughter in full screen like they do with slaughterhouse footage, you do get to see a close up of someone cutting in fur and removing pieces of flesh. Afterwards remains of the slaughtered pup are shown.
- The cremation of a dog can be shown in graphic detail.
- Staged footage is shown of a woman who gets attacked by a giant leech, a man attempts to remove the leech by cutting it. Green liquid can be seen, they use a blow torch to burn the leech off. It is implied that the girl died.
- Burn victims and other people with medical problems are shown, the narrator tells that in some countries they use iron rings which are heated to cure the patient.
- Staged footage is shown of a tribe ritual in which a member of the tribe gets stabbed and has his heart cut out.
- A short clip of a small child which suffers from cancer can be shown, this could upset sensitive viewers.
- Staged footage shows a raid during a satanic ritual in which human body parts are sold, the decapitated head of a man can be seen including some other body parts which are found in the nearby woods. A video clip is shown in which the satanists stab a man to death and slit the throat of another man (the last bit of footage looks more like something for a low budget gore-film).
