- One very undetailed topless scene.

- EXTREME graphic violence throughout the entire film. This film has incredibly realistic and truly graphic violence and the film's violence includes body parts expanding, organs being expelled, showers of gore raining all over the place, and many more.
- The main character Kenshiro is incredibly violent and relentlessly brutal. He fights enemies by poking their pressure points and the foes can be seen expanding to ridiculous proportions and then exploding into a thousand pieces like if they were balloons, and even if the most graphic parts are generally censored, all of these instances are clearly visible nonetheless and they're very, VERY gory. This is clearly nothing compared to the anime series, and it's very realistic and disturbing.
- The second one flies into the air and hits some rubble head-first.
- All of the description listed here occurs in the first act. There are numerous other moments of graphic violence in the film, and they're as gruesome and disturbingly violent as what happens before.
- Large amounts of gore and blood. Heads are constantly exploding (sometimes entire bodies explode), men are sliced up, and blood splatters all over. The footage often gets blurry and psychedelic during very graphic moments in the US release, but what happens is still very visible, and is super-violent and gruesome.
- The very beginning of the movie portrays the effects of a nuclear explosion with people's skin melting and their eyeballs falling out.

- While I wouldn't consider this frightening, it is very violent and intense. Many people are killed in extremely graphic fashion even though some of it is covered with a filter, but the scenes are still visible.

Violence & Gore
- In the beginning of the film, Kenshiro grabs a man by the head and squeezes his skull like a grapefruit, causing him to scream in horrible agony and soon, the pressure causes the man's eyeball to violently pop out and then he's dropped, where his head explodes off-screen with lots of blood splattering everywhere, and as the man's goons look on in shock, we see Kenshiro's hand covered in blood.
- Kenshiro hits a man in the face with his elbow, then sends another one flying into the air with his knee and kicks another in the throat, snapping it back. The three men are then shown suffering immense pain, and the montage shows us the following:
- The first one suffers from an unbearable pain as his left temple bloats into a tumor-like mass, as another one forms around his head, and eventually his entire skull is sent flying out his head, with blood splattering everywhere. This happens at a fast pace.
- The third one is shown to be headless and his legs rise up stiffly and then, his head and neck explode, sending a shower of blood fountaining and spraying out in an over-the-top fashion and his body violently twitches in an involuntary manner as some of his blood spurts on a boy witnessing this, horrifying him.
- A gang of criminals threatens some survivors to give them all what they have or else they'll die, and the leader laughs maniacally as he threatens to twist a young girl apart, only for Kenshiro to enter in action by kicking a brick into the leader's mouth, which he crushes with his teeth and soon a confrontation ensues, where the following happens:
- Three of the gangsters run into Kenshiro and try to harm him, but they only take off his cloak and then attempt to harm him, but he swiftly reacts by punching them and then, two more gang members run in only to be hit and another runs in as well, only to suffer the same fate. The consequences are then shown.
- The head of one of the gangsters, bloats to a ridiculous volume and then it explodes, with a huge explosion of white blood splattering on an area. Two other gangsters are hit by Kenshiro along with two more. Then one gangster suffers and the back of his head violently blows up, splashing blood and brains everywhere while the leader looks on. Then two members' heads explode in the same way, with more blood, gore and brain matter splattering all over the place.
- The final gang member then tries to kill Kenshiro, but he reacts by punching him in the face, flinging him into the air and landing on the leader. The back of his head then bloats and eventually explodes, sending tons of blood and gore splattering everywhere as we see his brains, eyeballs and tongue getting expelled before the camerca cuts to the leader's face being splattered in blood. The deceased member then falls down to the ground.
- The leader is then faced by Kenshiro, who demands him to put the girl down, only for him to reply by saying that he'll kill her because of his men being killed. This causes Kenshiro to go berserk and enter in a fit of rage, yelling as his top rips itself apart, and Kenshiro performs the Hokuto Shin-Ken on the leader, pummeling him with a brutal high-speed barriage before knocking him to the ground with a punch to the forehead.
- The leader then falls to the ground, sending the girl falling to the ground as Kenshiro catches her. The leader then rises up, attempt to destroy Kenshiro, only for Kenshiro to tell him that he's dead, and then, we see him look upwards, and his brain flashes, causing the leader's body to bloat and then his internal organs are misshaped, whereas this happens to his body, eventually causing him to explode, expelling his organs and splattering blood everywhere as he collapses.
