Sex & Nudity

- Simple stores tend to have sexual names but the products they sell aren't sex related.
- You can see women's underwear if you look up their skirts.
- In multiplayer mode, scantily clad girls dance suggestively.
- A mascot wears a costume which's the shape of a scrotum. There are toys about this mascot as well.
- Your character can pee in peoples mouths causing them to Vomit/Puke/Throw up.
Violence & Gore

- The Postal Dude can freely kill anyone with a variety of weapons, from firearms to explosives to melee-based weaponry.
- The game is incredibly gory; bloodshed, decapitation, dismemberment and complete bloody obliteration are very commonly seen should the Dude go on a rampage however due to the dated graphics this is nowhere near as realistic as other games such as DooM, GTA, Cyberpunk and Red Dead Redemption 2.
- Animal abuse also occurs (e.g. the Dude can stick a cat rectum-first onto a gun's barrel 5o act as a silencer, he can freely kill dogs, pigeons, elephants, etc.)
- When you kill someone blood splatters on the wall and they fall onto the wall.
- The Dude can set people on fire, which burns their skin off. Very graphic in approach.
- Several missions require the killing of police officers and soldiers.
- All of the above, however, is purely at the player's discretion; it was the developers' intent to have the player choose exactly how violent the Dude acts, as he/she does not NEED to kill anyone to complete the game's objectives. However, it makes the game more difficult to complete as many missions involve buying certain items, and money is very scarce in-game. Thus, killing/fighting is at times a necessity (and much more fun).
- The computer-controlled NPCs will often be seen fighting each other with no input from the player. One (apparently scripted) scene the player may often come across is a female being harassed by two cops. She attempts to escape, and is chased down by the cops, only to either be shot or beaten to death by them. The player can only prevent this by killing the two cops.
Profanity

- If you use cheats or commands, The Dude will insult the player (b.e. Sissy)
- Moderate amount, words like "fuck" are used, as well as other profanities.
Alcohol, Drugs & Smoking

- Players can smoke crack cocaine causing their health to raise, and also smoke catnip to go into slow motion. Your character also becomes addicted to the crack and loses health when he doesn't smoke any for a while.
- One location the player can visit is the home of a drug dealer who is seen selling his wares.
Frightening & Intense Scenes

- Share The Pain Expansion is, by any means, very gory. You have lots of weapons like machete, katana and others to hack people in various bloody ways.
- Explicit and dangerous amount of blood. Very intense game.
