When Worf is hit by the barrel at the beginning of the episode, he falls with his two arms in front of him, his left hand next to his head. In the next shot, his left arm is resting along his body, with his hand close to his knee.
During the operation, when the nurse applies the cortical-simulator to Worf's forehead, his ridges give way, revealing the foam rubber prosthesis.
In Sickbay after the accident, Dr. Crusher tells Worf that the falling barrel shattered seven of his vertebrae and crushed his spinal cord. However, during the operation, when Worf's spine is removed and placed into the genitronic replicator, it looks remarkably intact and shows no damaged vertebrae.
After Picard tells Beverly that Worf will kill himself unless he can undergo Dr. Russell's operation, Beverly angrily tells Picard that it won't happen "in her sickbay" and that she'll post guards around him before she lets that happen. However, later on, Riker is permitted to visit Worf with the Klingon ceremonial knife that would be used to perform the suicidal ritual, and he is even permitted to leave the knife on Worf's person before leaving sickbay.
