Dr. Gilani and Dr. Frost are examining two chest x-rays, the x-ray on the top is backward. The heart can clearly be seen in the right lung field. While it possible the patient has dextrocardia (the heart on the right side of the chest cavity), this is a very rare condition. So, it's more likely the x-ray is backward.
