- In Portland, Oregon, at St. Theresa's, Dr. Louise Hastings is working in her office at the asylum. The office door opens, and the keys on Hastings keyboard type by themselves. The glass on a nearby bookcase shatters and alarms go off, and Hastings curses. She leaves the check out the alarms, as the words ""No mercy" keep typing on her computer screen.
A guard tells Hastings that two inmates are missing, and they have no idea how they got out. Hastings goes to the secure ward door and tells the guards to open it. Once they do, Hastings and the two guards go to a storage closet and open the door. Another guard is inside, a broom driven through his chest. Hastings tells the guards to lock it down, and they run off to obey. She then looks at the door of a cell down the hallway.
In Salem, Oregon, Mrs. Cavallo yells at Sister Gabriella Rosetti to get out. Gabriella says that she's just trying to help but goes. Mr. Cavallo goes out with her and apologizes, and Gabriella says that her associate Daimon Helstrom will be there shortly to pick her up. The man says that he called the archdiocese and confirmed that Daimon isn't a priest. Gabriella concedes the point, and Mr. Cavallo asks her how it's going to work. She only says that she's fairly news.
Daimon pulls up in his car, on the phone complaining about how they sent another nun-in-training. He tells the person he's calling to call them when they can from St. Theresa's. Gabriella introduces Mr. Cavallo, and how they were just discussing his son Archer. Daimon has them take him in, to where Mrs. Cavallo is sitting at the kitchen table, drinking. Mrs. Cavallo says that she doesn't care who Daimon is, and Gabriella tells Daimon that the "problems" begin several months ago. Daimon already knows the details, and goes upstairs to Archer's bedroom, and tells Mr. Cavallo that he doesn't need any religious artifacts for his task.
After using the bathroom, Daimon goes up to Archer's bedroom and tells Gabriella not to come in no matter what she hears. He puts his hand on the bedroom door for a moment, then withdraws it in shock. Daimon then goes in, closing the door behind him. Archer is lying beneath his bed, and mutters about the "thief" coming and destroying in Latin. Daimon recognizes Latin, and the demon possessing Archer refuses to give his name. The demon tells Daimon to tell Archer's parents that his body needs sustenance or it will rub their walls with excrement.
The demon crawls out from beneath the bed, and Daimon takes out a vial of holy water and sprinkles it on the demon. It writhes in pain, then stops and looks up. Archer is apparently back in control of his body, and Daimon says that the "holy water" just came from the Cavallos' bathroom. Daimon dismisses the Latin as a trope, and says that secrets lose their power once they're exposed... like the demon.
Archer tells Daimon to get out, and Daimon says that he will once Archer tells his parents that it was all an act. The boy insists that his "power" is real, and Daimon tells him that real power is in his heart and brain. Archer concentrates and the furniture telekinetically slides back, and a circle of fire surrounds them... done by Daimon. As Archer stares around in fear, Daimon extinguishes the flame and Archer backs away from him in terror. He tells the boy to clean the excrement from the walls with bleach and leaves.
Daimon and Gabriella leave and the Cavallos accompany them, and Daimon tells the parents that their son isn't possessed but is an asshole. He says that Gabriella can refer them to a family therapist and meanwhile they should stop drinking and be better parents. Once the Cavallos go back inside, Gabriella tells Daimon that he can't be insensitive to everyone they meet. Daimon doesn't care and says that he can't spend his time responding to false alarms. He gets in his car and drives off.
At an auction house in San Francisco, Ana Helstrom walks in and Chris Yen tells her that "he" is there. They go to the front and Chris says that he can see the bidder's heart on a computer. The auctioneer opens the bidding on the next item at $7 million, and "he"--Bidder Edward, Edward Tate--starts the bidding. The bidding continues, and Chris says that he'll send Edward up when it's over.
Later on the roof of the building, Ana talks with Edward. They flirt, and Ana talks about how weapons of war have always been an interest of hers. Chris secretly watches from the side, and Ana tells Edward that he overpaid for Napoleon's Sword. Edward doesn't care, and Ana says that she's heard Edward likes twisted. He tells her that she has no idea, and Ana says that she does and she knows how he tries to avoid any public attention. She tells Edward that she procured the sword just to get Edward there, and says that she knows his secret. Ana whispers the name of the demon possessing Edward into Edward's ear, touches his chest, and senses how he kills women to steal their life force.
Ana tells Edward that he's killed seven women, and she's taking it all back. She touches his chest again and Edward gasps in pain as Ana draws the women's life force from him. Ana shoves him off the edge of the building onto the street far below, then walks off.
In the "City of the Dead" in Colma, California, the asylum nurse Keith Spivey breaks into a bier in a mausoleum as the two escaped patients calmly watch. Spivey pulls out a chain and tells the two observers to help him pull. They do so, opening the lid of the bier, and reveal a stairway leading down. The trio descend the stairs, and come to a series of runes carver on the wall. Spivey takes photos of them, then continues on.
The trio come to a snake inscription on a wall and Spivey takes photos of it, saying that he found it. The inscription is on a door, and Spivey and the two patients open it. The patients hesitate, and Spivey admits that he doesn't know why "she" chose them. Spivey starts to open it himself, and one of the patients slams his head into the inscription.
Spivey curses them, and tendrils from the door grab him and pull him onto it. As Spivey bleeds out, one of the patients--Liddle--tells him that's why they were chosen. Spivey screams in pain and dies.
At Gateway University in Portland, Damion gives a lecture to students about how humans fear and then are rewarded, and that leads to moral behavior. Hastings appears in the door out, and Damion sees her. He soon ends the class and meets afterward with Hastings. As they walk, Hastings tells Damion that Gabriella told her what happened and the Cavallo family will receive counseling. Gabriela was highly recommended by the Vatican, and is there to help Damion. Damion says that it's not the same with Hastings, who knows what he deals with but Gabriela doesn't. He insists that Gabriela isn't ready, but Hastings refuses to argue.
Hastings tells Damion that two patients escaped from the secure ward with Spivey's help. She's sure that it was Victoria Helstrom, Damion's mother, and Victoria only responds to Damion. The dead guard, Stephen, had just come back from his honeymoon. Damion tells Hastings to tell Stephen's family that his funeral costs will be covered by an anonymous donor, then walks off.
Ana meets Chris at their base, and he complains about all the "damage control" he had to do on his behalf. She doesn't care, and admits that she was having sex to relieve her stress. Chris assures her that there no mention in any media outlet of Ana being at the auction, and says that she's reckless. Ana claims that Edward "accidentally" fell, and discovers she missed a call from their boss, Caretaker. Chris says that Caretaker is off exploring ruins somewhere, and he mentioned a crypt in Colma. Ana immediately leaves, ignoring Chris, much to his dismay.
Gabriella goes to St. Theresa, where Damion and Hastings are in the security room reviewing video of Spivey. They figure he killed Stephen and broke out the two patients, and they all left together in a hospital van. Hastings tells Gabriela that there are no cameras in the incarcerated patient's room, and Gabriella realizes that the patient is Victoria. She wants to go with Daimon when he goes to see his mother, insisting that the Vatican needs to know about her. Daimon tells her that no one goes into Victoria's cell but him, and leaves.
Daimon goes to Victoria's cell and finds her lying on the floor, seemingly unconscious. He finally addresses Victoria, asking if she can hear him. There's a symbol drawn on the wall, and Daimon asks Victoria what it is and what did it say to Spivey.
When Victoria remains unconscious, Daimon kneels next to her and says that he knows Victoria is inside her body. He talks about how when he was 9, he fell from a tree and Victoria told him to push through the shock and breathe. Victoria looks at him and says his name, and says he never should have been born.
Daimon realizes the demon is still possessing Victoria, and demands to speak to his mother. The demon says that Daimon's memory is tainted by sentiment, and Victoria wants Daimon to "grow a spine" so he wouldn't be an albatross around her neck the rest of her life. The entity talks about how Victoria drank cheap wine trying to get rid of the unborn Daimon.
Daimon ignores the taunts and demands to know what the symbol means. He continues questioning the demon, asking where it sent Spivey and the two patients. The demon says that she sent them to "see an old friend", then tells Daimon that they're done and lies back down.
Ana arrives at the mausoleum and meets Caretaker, who tells her about the secret cavern beneath it. She asks where he's been, and Caretaker tells her that he was in Texas. Caretaker gives her a pair of boots to put on and leads her into the caverns. There are piles of human meat and bones in front of the door, and Caretaker says that the inscriptions are ancient demonic.
A one-eyed skeletal being is hugged to the door by the tentacles, and Caretaker says that it's a Keeper: a demon guarding something powerful, that is now free. Ana wonders if it was "him", and Caretaker asks her for her opinion, because "he" has taken a lot of forms over the ages and regenerated himself.
Ana touches the book but gets nothing off of it. Caretaker suggests that Ana reach out to her brother, but she refuses. Caretaker goes to call in men to seal the place off, and Ana stares at the skull. When Caretaker breaks the spell, she turns to him and vaguely relates what he said while she was entranced. Ana says that she wants the skeleton, pulls it off the body, and walks away with it.
In Hastings' office, Daimon tells Gabriela that the symbol in Victoria's cell is demonic in nature. He tells Hastings what Victoria said, but has no idea who the "old friend" is. Gabriela wants to enter the cell as an impartial witness, and Damion and Hastings warn her that Victoria is dangerous. When Damion dismisses Gabriella's education in the supernatural, Hastings tells them to both back off. Gabriela doesn't care for Damion's continuous insults and leaves.
Hastings tells Damion that he owes Gabriela an apology, and he sarcastically agrees. She insists that Gabriela is trained to view exorcism through the eyes of a skeptic, and they need Gabriela's help. Hastings says that she raised Daimon better than what he is, and Daimon admits that she did. Daimon is worried that he can't help Victoria alone, and figures that he needs Ana. The last time Daimon and Ana talked, Ana said she would never come to Portland again. Hastings suggests that's for the best, and Daimon walks away without a word.
Later, Daimon sends a text to Ana asking if they can talk. Ana is at the base looking at photos of skulls on the Internet. She sees the text and remembers their childhood, and her watching a chest-scarred Daimon lying on the floor. Ana ignores the text and goes back to work.
As Daimon gets a tattoo on his chest, he remembers his and his sister Ana's childhood. The tattoo artist, Rose, asks Damion why the tattoo ink just disappears when she tries to tattoo Daimon's chest scar. He tells her to keep trying.
Ana gives up searching in frustration, cuts her thumb, and smears the blood on the skull's forehead. She then puts her hand to it, trying to get an impression. An eye pops out of the single socket for a moment, and Ana withdraws in shock. As she watches, her blood flows into a symbol on the skull.
The next day at the City of the Dead, Caretaker digs a grave and buries the patients' remains. He notices a piece of paper in the remains and studies it, and finds it's a tag from St. Teresa's marked "DFMM".
Parked outside the Cavallo house, Daimon calls Gabriela's phone and leaves a message apologizing to her for his tone. Mr. Cavallo and Archer drive off.
Hastings approaches Gabriela at her car and suggests that she forgive Damion. Gabriela isn't convinced, and Hastings asks where she's going. The nun says that her superiors are concerned that Hastings allows Damion to perform exorcisms, when he has unidentified powers he may have acquired from a source of evil: Victoria. Hastings points out that exorcism requests have gone up tenfold in the last year, and Gabriela demands to see Victoria and know what's really going on. The doctor gives Gabriela a pass, and says that there's something on her desk and Gabriela can take it to the Vatican with her if she wants.
Gabriela goes to Hastings' office and finds a binder on the desk. It contains files on Damion as a child, and how he lived with Victoria, who was diagnosed as schizophrenic.
Damion sits in his classroom and circles the same word, on a notepad.
Gabriela turns to a file on Ana, and an article about how her father kidnapped her when she was young. There are more articles about how the Helstroms' father killed a number of people.
Ana takes the skull to storage.
Victoria lies in her cell, staring off into space.
Ana puts the cloth-wrapped skull on a shelf and leaves.
Gabriela finds a journal with Victoria's words written, saying that the writer won't let "him" take Ana.
After the students in his class leave, Daimon hallucinates the paper he's writing on bursting into flame.
Hastings enters her office, and Gabriela tells her that Damion and Ana's father was a serial killer. The doctor insists that Victoria wasn't insane but cursed for discovering the truth. Hastings says that Ana is in San Francisco and hasn't been back to Portland in years. That's part of why Damion and Ana have never gotten along. She says that what happened to the mother and children wasn't a random family tragedy, but evil. They live with that every day.
Ana sits in the storage room and remembers her childhood, and being taken away from Victoria and Damion by her father. She stares at a coffer, then opens it and looks at her childhood keepsakes inside. Ana starts to reach for them, then slams the lid shut and pushes the coffee away from her.
Gabriela wonders why Damion can't reach his mother if he has his "gift", and Hastings admits that they don't know. Hastings has a coughing fit and spits out blood, then runs out before Gabriela can see it. Gabriela takes the badge and goes to see Victoria. She enters the cell and finds Victoria standing behind her. Gabriela insists that the demon in Victoria doesn't scare her, and Victoria telekinetically slams Gabriela against the far wall. Victoria grabs Gabriela by the throat, and Gabriela demands to know what the demon's mission is and who it answers to. Victoria insists that she answers to no one and slaps Gabriela across the room.
Hastings checks the security footage and confirms that Gabriela went into the cell, and heads there.
Gabriela pulls out her rosary and chants in Latin. Victoria isn't affected and tells the nun that she couldn't comprehend the answers to her questions. The demon slams Gabriela against the wall and says that the demons lost their war long ago to the "oppressors". They were given a verdict of "no mercy", and that's what Victoria gives Gabriela now.
Gabriela punches Victoria, and Victoria telekinetically slams the nun against the ceiling. Damion and Hastings run in, and Victoria drops Gabriela to just above the floor. Victoria charges at Damion but hesitates before choking him. Once she grabs Damion's throat, he touches her head and the demon says that he's only hurting Victoria. Damion tells the demon to let Victoria go, and the two of them are pushed away by the forces between them.
As Damion and Victoria recover, Damion remembers himself as a child, scrubbing the bathroom floor with a toothbrush. He also remembers his mother comforting him. Victoria, briefly regaining control of her body, tells Damion to run. Hastings pulls Damion out and they leave the cell with Gabriela.
Later in Hastings' office, Hastings pours Gabriela a drink. Damion says that whatever the beings are, demons or entities, it doesn't matter because they don't belong on Earth. They feed off the suffering of humans, which gives them pleasure. Damion explains to Gabriela that the symbol from Victoria's cell is an ideogram that means "consumption".
Gabriela is shocked by the pure evil in Victoria's eyes, and Damion tells her to remember the Biblical inscription against demons and drinks her whiskey before leaving. He goes to his parked car, and finds Ana waiting for him. Ana asks him to tell her that Victoria is dead.
A trucker leaves a gas station and finds a man, Alex Tilden, sitting in his cab. Alex's face is partially torn open, and he lunges at the driver.
