At approximately 9:15 into the episode one of the doctors says, "Dr. Clancy you got a minute"; however, at that point in the show Jim Clancy has died and is in the body of Sam Lucas. Therefore, the doctor should have said, "Dr. Lucas you got a minute?"
When the Ghost Surgeon is operating, a heart monitor with red LED displays is seen. The date of the surgeon's death is found out to be 1959; however, the first practical LED was invented by Nick Holonyak, Jr., in 1962.
After Melinda gets off the phone with "Jim" who took over Sam Lucas' body in season 4, you hear a line in the background where they call for Dr. Clancy, not Dr. Lucas.
At approximately 09:15, Jim/Sam is in the stairwell talking on the phone to Melinda, and just as they are wrapping up, another doctor says, "Dr. Clancy, got a minute?" No one knows he is Jim Clancy; at that point in the show Jim Clancy has died and is in the body of Sam Lucas. Therefore, the doctor should have said, "Dr. Lucas you got a minute?"
- Jim/Sam's hospital white coat (at least in the first few episodes of S5) is embroidered "S. James Lucas M.D." - so his colleagues would probably know him as "Jim", but "Dr. Clancy" looks like a definite Goof.
