- Boris flies Hank to another country, where he and another doctor research potentially life saving gene therapies. Back in the Hamptons, Hank's temporary replacement, Dr. Emily Peck, rubs Divya the wrong way.
- When Boris avails his private jet, Evan, with a prepared bag of HankMed marketing gadgets, insists to tag along with Hank but, unlike their supposed Manhattan destination, they're taken to Cuba, breaking the U.S. embargo. Evan tries making the best of opportunities, even without cash dollars, but a foxy Canadian pickup, who invites him diving, nearly breaks his skull. Hank is expected to supervise Cuban researcher Dr. Marisa Casseras's experimental drug test protocol and test Boris but finds Boris too eager to follow cautious advice, leading Hank to withdraw, indignant. Back home, Divya is not impressed with Dr. Emily Peck, the temp concierge doctor Boris hired. Though a whiz on paper, she's a commercial anti-Hank.—KGF Vissers

Evan shows Hank his recent marketing ideas (monogrammed carry bags, HankMed thermoses, Hankpop candy for kids) as Hank prepares for a one-day trip to New York City to help Boris with a genetics trial. Hank accepts Boris' private jet, on which Evan ultimately convinces Hank to let him tag along. Mid-flight, Hank realizes the journey is longer than a trip to the city. They land on a tropical island where Boris greets them: "Welcome to Cuba."
Boris tells Hank he felt the need to trick him in order to get him to Cuba, an embargoed nation. He has arranged for a doctor to cover Hank's work for the weekend. Boris tells him Dr. Casseras (Marisa) has diagnosed him with a hereditary mutation that will eventual destroy his brain, muscles and bones. Marisa has a clinical trial in town and Boris wants Hank's help with the pre-trial testing.
Back in the Hamptons, Divya meets Dr. Emily Peck, Hank's replacement for the weekend. Emily shows Divya a note from Boris then makes a crack about her being late. It is clear Emily will regard Divya as an underling rather than a colleague.
Evan is upset Hank won't have any time to hang out with him while they're in Cuba and goes exploring by himself. At the beach, he finds a man selling scuba trips. When he finds he lacks cash, a Canadian woman named Mindy pays for him as she needs a "dive buddy." She is in Cuba with some girlfriends who are more interested in working on their tans than scuba.
Boris brings Hank to Marisa's facility. She thanks Hank for encouraging Boris to explore genetic testing but doesn't seem to like Hank presence.
Divya assesses Emily taking selfies of herself with clients to be self-indulgent and her overall attitude (around listening to her own podcast when alone with Divya) demeaning.
Hank asks Marisa when he might see the blood work results. Spotting a picture on her desk, he congratulates her on having Boris as a benefactor. Thinking him suspicious, she doesn't seem to care for Hank's concern over the safety of her trial - presuming he's a typical American concerned with the almighty dollar over health - but Hank states his concern is over Boris not endangering his life by trying to save it.
While scuba diving, Evan and Mindy seem to connect. The boat calls out, sighting a Portuguese man o' war. Evan and Mindy manage to safely swim back to the boat, but once on deck, Mindy accidentally whacks Evan in the head with her air tank and he plunges nearly unconscious back into the Gulf of Mexico.
Summoned to the beach to attend Evan's head injury, Hank snagged Evan's bag by mistake, which holds HankMed tchotchkes rather than the needed gloves and suturing kit. Hank scraps pieces of a beach bag to use with hair braids to close the cut on Evan's scalp. With all put right, Mindy and her friends leave.
Before her appointment with Alan Ryder, Emily shows no interest in Divya providing her with personal experience information. Alan continues to have migraines so Emily orders an MRI with a battery of other expensive tests. Divya thinks it is a mistake, but Emily shows no interest in anyone's opinions but her own. Leaving, she orders Alan to pee in a cup then hand it back to Divya.
Hank and Evan have just finished dinner with Boris. Evan is frustrated things didn't take off with Mindy (and that, due to his head injury, he's the only one in the Caribbean with a cocktail ban). Boris strolls them along to a square where Marisa hands out the medications he had had delivered to his house last episode (for which Eddie signed and Hank scolded Boris).
Divya goes to see Jill (who argues on the phone with a Realtor over selling her house). She asks about Emily, and Jill pulls up her sparkling resume on the hospital server. The lack of any red flags disappoints Divya.
That night, seeing Marisa and Boris dancing romantically interests Hank. He is ethically troubled over the apparent relationship between Boris and his doctor.
While performing a biopsy, Hank confronts Boris about Marisa. Hank isn't thrilled over Boris funding her research - but maybe Boris feels it's none of his business (to which Boris seems to agree).
While Evan works on purchasing some cigars, he runs into Mindy. She seems happy to see him but already has plans for the evening. Evan trades his cell phone for the cigars. A grim figure watches this whole transaction.
Alan's test results come back normal, but he is frustrated his headaches won't go away. Emily gives him an injection of Botox to help what she believes to be tension headaches, and offers to use the remaining Botox for its more popular use: getting rid of wrinkles. Alarmed, Divya intervenes: HankMed does not advocate or do cosmetic surgery to patients. Alan agrees to go ahead, but not so much that it'll make him look like Mickey Rourke.
Hank tells Boris his favorable lab results make him a strong candidate for the clinical trial, but Hank wants to do research trials while Boris wants to charge ahead immediately; however, Boris seems to agree to wait.
At a party, Evan tells Hank he thinks the Cuban government might be following him. Hank meets a pretty girl. Mindy comes running into the bar to tell Hank her friend Susie is in bad shape. She's having complications from several plastic surgeries and Hank orders them all to the hospital.
Thanks to Evan, Hank pretends to be Susie's cousin to help the local doctor. They determine she has the bends (caisson disease) due to a combination of anesthesia and scuba diving. They take her to a hyperbaric chamber.
Mindy thanks Evan for helping her friend. She says she's going to have some free time coming up. Evan is disappointed he's supposed to leave in the morning. The two kiss and leave to spend his last night in Cuba together.
The next morning Hank arrives as Mindy leaves (wearing a HankMed shirt). Evan takes Hank's phone to go buy more cigars, the goal being to give them to their top clients back home.
Through one of the windows, Hank catches a disturbing sight: Boris starting the clinical trial. He tells Hank he "made a decision." He thought Marisa's plan made sense and so - choosing between following Marisa and increasing the risk or following Hank and delaying the benefit - he chose to "do" rather than "wait." He asks Hank to stay through the trial. Though much concerned, Hank says no and catches up to Marisa in the parking lot. Hank tells her it's clear that she still loves him. She seems to agree and is happy to be treating him again, even though the situation isn't ideal.
Divya hear's the door slam, but it's not Hank. Emily gives Divya notes to transcribe and her card for the next time they need help. Divya rips it up and tells her she isn't professional. Taking her notes back with a smile, Emily leaves for her doubles match with client Alan.
Before boarding the jet, Hank tells Boris he hopes everything works out for him. Hank will continue to look for a cure. Evan is not aboard the plane yet.
Evan looks for his cigar dealer unsuccessfully. In an alley, a man grabs Evan and throws him into a car which then peels off as the episode ends.
