- Tommy's first day back on the job is a busy one as the crew races from call to call. By the end of the shift, the guys are all exhausted, but it's Lou who feels it the most.
- At home, Tommy pours out all his bottles of booze. He tells Janet that he and Colleen are quitting smoking and drinking. She grabs wine from the fridge to pour out as well. He doesn't think it's necessary because wine isn't booze, it's a "delicacy." Colleen joins them. "What's better with Cheerios? White or red?"

Tommy calls Damien just to make sure the guys aren't planning any big party for his return. The house is calm but Damien acts disappointed and promises to break the bad news to the guys. He doesn't, and instead rejoins them putting a puzzle together.
Tommy comes in looking for his welcome and instead finds a burping contest in the kitchen. Needles forgot today was the day Tommy was coming back. Tommy goes looking for the strippers. Everyone is uninterested that Tommy is back. He can't believe there's "no cake, no girls, no hats, nothing."
The non-celebration is interrupted by a call. They race to a scene half way between their house and the West Side Wild Men and both arrive at the same time. They argue over who's getting the call of a OD.
When the guy passes out and pukes they start arguing over who has to take it.
They resolve the situation by putting a bag of ice down the guy's pants and reviving him.
Sean and Mike get tried of getting ragged on about being idiots by Black Shawn, but struggle to come up with a plan to prove him wrong. Sean suggests charity work, but not New Orleans because Brad Pitt has that covered. George Clooney's got starving children in Africa and Matt Damon has cleam water. Back to the drawing board.
Tommy is talking to Franco when Janet calls Franco. He calls her "Jan" and gives her fashion advice.
Damien tells Needles he might have brought Tommy back too soon. But Needles is more worried about Lou's "gock," his big gut that hangs past his (rhymes with gock).
In the truck back, Mike and Sean go back to trying to come up with a charity idea. Black Shawn mocks them, leading to an in-truck fight.
They return to the house, where Cousin Mickey and Teddy are waiting for Tommy with an expensive bottle of really nice Irish whiskey. He tells them he's only drinking wine now. They tell him they've decided he's a lost cause and are cutting him out of their lives and leave him with the whiskey.
There's another call. In the truck they whine about it being the eighth call in a row.
At the scene, they have to get a guy out of an open street grate. Sean and Black Shawn are lowered down. Lou doesn't look too well, leaning against the truck. He holds his arm, then bends over to breathe. No one notices.
They raise the guy from the bottom of the hole, but decide to leave the Sean/Shawns, down there. Tommy notices Lou sitting off to the side and checks on him.
He thinks he pulled something getting off the rig. He tells Tommy they're both getting older and gives him his annual "take the Lieutenant's Exam" speech.
Out of the hole, Shawn apologizes to Sean for calling him an idiot. They commiserate over dating Gavin women. Shawn suggests Sean do something about 9/11 related illnesses, like the cancer he got.
Janet calls Tommy at 6:17pm to let him know Colleen is starting her fourth glass of wine. She asks to talk to Franco. Everyone watches as he guides her through her own house looking for something. He calls her silly then tells Tommy it was Vaseline for the baby's rash. (One of the places he told her to check was the bedside table.)
Sheila summons Tommy to her car. Damien isn't calling or texting her. Tommy says everything's cool. She locks him in the car with her. Tommy tells her Damien did a solo save and the department is going to give him a medal. She's mad, then decides it's not that bad because he only got the Gavin hero gene, not the alcoholic-sex addict-arsehole gene. Tommy tries to tell her she smothers Damien, she fires back by insulting his mother and in defending his mom he describes his dad as a jerk firefighter with a drinking problem and three girlfriends on the side. Realizing he just described himself, he decides she's doing great.
Damien knocks on the window and wants to know what his mom is doing there. She says it's unrelated to him but he doesn't believe her. He kicks Tommy out of the car to talk to his mom, but we can't hear what they're saying.
Then Damien leaves and Sheila says everything's OK, but she's acting weird. She says she realizes Tommy tried his hardest and she "releases" him from his Damien obligations.
They get another call to respond to.
The fire is in a church. Two people are still inside. Lou starts to go inside but grabs his chest. He goes in anyway. They find a nun upstairs and carry her out. The priest is inside trying to save something. They get him out, too. Tommy goes to save whatever it was.
Outside, he gives the priest (Peter Gallagher) a three-foot Virgin Mary statue. The priest is thrilled it was saved because it's hundreds of years old and direct from the Vatican. He takes Tommy to task for a small chip in it.
One of the guys from the West side house comes over and makes a crack about the statue, saying he hoped Tommy copped a good feel. Tommy responds by punching him. The priest appreciates it and says Tommy beat him to the punch.
Back at the house, Needles has pizza waiting for them. He commends them for 35 calls in one shift. He presents Tommy with a paper bag with a bow on it.
It's a Ladder 62 kevlar vest.
Damien comes in, saying something's wrong with Lou. They all go look and find him face down on the floor, eyes open and not breathing.
