- The game contains some sexual material: camera angles highlighting characters' underwear; characters making sexual remarks (e.g., "I've heard women are the best just before they're ripe, but this is beyond my expectations" and "Ufufu, put that lewd body of yours to work and you'll never go hungry!"). In one sequence, players must slap away groping robotic hands from a bound female character. A protracted fantasy sequence depicts a character's hands grasping sheets; sexual moaning sounds can be heard.
- A little girl is said to have been molested by both her parents and their friends. See Frightening and Intense scenes category for more info.
- One character is fairly perverted and you can see her "fantasy" scenes every so often. She is scene grasping the sheets but nothing explicit is seen. Some moaning sounds can be heard and the scenes are extremely lewd.
- One character states that he "likes them as young as possible" (implying he is a pedo)
- There a scene where the main female character gets kidnapped and get trapped in a groping machine, In the mini-game, the player must slap always the groping robotic hands from the main female character body. If failed, the screen will be splatted by white stuff (resembles as cum) meaning the main female character came. And basically enjoyed it and now basically "mind-breaked"

- As players move through the city, they can switch between each character and use hacking guns and scissors to destroy robotic bears. A handful of cinematics depict additional acts of violence: characters impaled by bolts; robotic bears stabbing, shooting and killing civilians; a character's legs slashed with knives. Blood is frequently depicted on and around corpses.
- A character slashes another's knees with her scissors. However, like the other three games in the franchise, all the blood is a bright neon pink color.

- The words "fuck", "shit", "bitch", etc are heard in dialogue.

- One character is a smoker and apparently drinks a lot.
Frightening & Intense Scenes

- The Game's villains are children who have grown a hatred of Adults and begin purging them because of their abusive upbringing. This includes Physical Abuse, Verbal Abuse, Neglection and even Sexual Abusive. While none of these acts are ever shown onscreen, they are explained and implied in a way that is still very disturbing and can be too uncomfortable for many players.

- The adults becoming a rioting lynch mob killing every Monokuma robot and child they see, regardless if they're brainwashed or not. They all turned on Komaru and Toko when they realized what they have become.

- A kid named Nagisa, His parents inject drugs into him to make him stay all up night studying.
