- There are two sections of the game where Madeline has to run away from something chasing her, in both of them you can jump on their heads to slow them down. There's no blood, and it's no worse than a normal Mario game.
- There is one, long, boss fight where a character attacks another character using lasers.

- At one point, Madeline may say that she drank to help her depression, but this dialogue can be avoided.

- The plot deals with Madeline's feelings of depression, so it gets quite emotional at times.
- In one scene, Madeline has a panic attack. This is abstractedly depicted as large tentacles reaching out towards her with deep intense music playing in the background. She then calms herself down by imagining controlling a feather with her breathing. May be disturbing, especially for those who suffer from anxiety and panic attacks.
