The plot of Peter getting misidentified as transgender intrigued me. And once you see how it happens, it makes sense. As you'd expect, he takes advantage of this situation. One funny moment is when he goes around insulting people under the guise of being a woman, since he's less likely to be hit that way. Chris Brown hitting him anyway was expected, but still made me chuckle. I also liked the callback at the end where Quagmire gets back at Peter for what he said about Sully. There's a montage bit with all kinds of different hairstyle choices, where instead of going with the most beautiful, he gets all hairstyles at once.
Brian as usual tries to be as mindful as he can, but fails when he can't get the LGBTQ acronym right. There were also some other funny miscallenous gags such as the cats in space and Joe getting a special sort of "ice cream" (even though it was really gross as well). And while I think they've done the diaper joke with Joe too many times, Patrick Warburton's delivery when he whispers "I've got poopies" to Quagmire is funny by itself.
The live-action cutaway with Bill Murray reacting to the unorthodox hospital procedures was weird, but still amusing.
There's a reason this episode has gotten backlash however, and that's the serious speech by Ida at the end. But I think people overreacted a bit. There was still a lot of politically incorrect humor throughout, only this time with a message to go along with it. It's not any different from You May Now Kiss The Guy Who, Uhm... Receives, where Lois learns to be tolerant towards gay people.
The ending with the Bob's Burgers family making a cameo was hilarious, primarily due to Bob's confusion over why they were included.
Trans-Fat isn't flawless. The jokes about "things women do" felt really forced at times, plus some other jokes were repeated twice for no reason (the man with the hats, Joe needing his diaper changed). Other than that, this is a good one.

I wasn't offended by the content of the episode, but it just wasn't that funny. I can excuse a bad storyline if the jokes worked, but no dice tonight.
The one bit with Peter constantly turning his back while the action went on in the game was the highlight though. That made me laugh the whole time despite its repetitive nature.
