The problem with this show for me was that every so often it was able to produce episodes of very high quality like this - but they were buried amongst some weak ones indeed.
Focusing on the positives though this is a very strong episode, up there with the best of the series because the main plot is engaging, Patricia Heaton isn't having to strain to make average material work, and there's some genuine heart.
They've definitely done the right thing switching to having plots span more than one episode, and showing more of Carol at home with Ashley Tisdale's character too.
It's just the supporting characters that let it down. I'm sure the actors will all go on to be in better sitcoms, but their stories often tank badly. The one here isn't too bad, but it's a stretch, even in a sitcom, to believe that they are competent doctors. And as for Caleb, well, I'd have ditched the character ages ago.
So yes, while overall a frustrating series, this episode shows the promise it has.
